- FAFSA - In the School Release section of your FAFSA, you must indicate the University of Miami, Coral Gables and the federal school code 001536. Submission of an incorrect school code may delay the processing of your financial aid application.

- Master Promissory Note (for both Unsubsidized and Grad PLUS)
- Original Document Affidavit
- Identity Verification Form - YOU MUST PRINT, SIGN THIS FORM IN FRONT OF A NOTARY PUBLIC, AND MAIL THE FORM TO:
